In recent years there has been increased interest in the tropical splenomegaly sn e and the subject has recently been reviewed (Pitney, 1968; Marsden and Hamilton, 1969; British Medical 7oural, 1969). Outside Africa pical splenomegaly syndromne has been described in the inhabitants of South Arabia (Fawdry, 1965), who anthropologically are caucasoid, in the Melanesians of New Guinea (Marsden et al., 1967; Pryor, 1967), who are negroid, and in Bengalis (Chaudhuri et al., 1956), who are caucasoid. No Caucasian (or "European") cases of tropical splenomegay syndrome appear to have been reported from the African continent, and at least one observer with long., experience in Africa states that he has yet to diagnose such a case (Lanzo, 1969). Clinical trial of malaria prophylaxis in tropical splenomegaly syndrome.

A controlled, randomized, double-blind trial of malaria prophylaxis in tropical splenomegaly syndrome showed a significant reduction in spleen size and an improvement of anaemia and symptoms in patients treated with antimalarials compared with control subjects receiving placebo. This study confirms the observations from West Africa and provides further indirect evidence for a malarial aetiology...

Hyper-Reactive Malarial Splenomegaly Syndrome (HMSS)

Hyper-reactive malarial splenomegaly syndrome (HMSS) is a massive enlargement of the spleen due to an exaggerated immune response to repeated attacks of malaria. Tropical splenomegaly syndrome (TSS) is the most frequent cause of massive tropical splenomegaly in malarious areas [1-2]. It is seen more commonly among residents of endemic areas of malaria. It occurs mainly in tropical Africa, but a...

Hyperreactive malaria in expatriates returning from sub-Saharan Africa.

The extreme presentation of hyperreactive malaria is hyperreactive malarial splenomegaly syndrome (HMS). Some patients present with a less pronounced syndrome. To investigate whether the degree of splenomegaly correlates with the degree of immune stimulation, whether prophylaxis or recent treatment play a role, and whether short therapy alone is effective, we examined retrospectively the medica...

Relapses after withdrawal of proguanil treatment in tropical splenomegaly syndrome.

After the remission of symptoms and reduction in spleen size while on proguanil therapy four patients with the tropical splenomegaly syndrome defaulted from treatment. The withdrawal of proguanil caused a recrudescence of original symptoms, splenomegaly, and a return of the initially raised serum IgM. Complete return to normal values was again effected with proguanil therapy. The role of the spl...
